SWAN making appointments

SWAN Senior Services is taking appointments for Benefit Access, formerly called Circuit Breaker, applications for 2013.
Applicants must have turned 65 years of age or older by Jan. 1 of last year, must become 65 years of age during 2013 or qualify as a disabled person.
Applicants need to provide their driver’s license, Medicare card, Social Security cards and a copy of prescription drug plan, if applicable.
They will also need to provide proof of income received in 2012, including Social Security. Income information must include bank accounts, investments and pension income.
For those who file a tax return, a copy of that return is needed also.
The eligibility guidelines are:
• 1-person household, $27,610
• 2-person household, $36,635
• 3-person household, $45,657
In addition to Benefit Access applications, SWAN Senior Services can assist with Medicare Part D Drug Plans, low-income subsidy applications, Medicare Savings Program, information on a Medicare Part B benefit, and prevention and wellness. SWAN also makes legal appointments for Land of Lincoln for those needing a Power of Attorney or simple will.
